Pre-Event Customer Questionnaire

 

Our Pixies will need some time to create and customize your event, so it may be several months before you receive a reply. We work on one month at a time and then in the order in which they are received. We are currently working on events for June and will start July soon. We only accept questionnaires for dates up to 6 months in advance.

If you have any questions, please email jane@giftsofalifetime.com If you have not heard from us and it is less than 3 weeks to the date you are requesting, please e-mail to confirm we received your questionnaire. If your email address is spam protected please make sure you allow emails from the above address. Please take the time to reply to our event ideas, even if your response is a 'no, thank you.' A response is appreciated more than silence. Individuals that do not give us the courtesy of a reply to the event idea presented to them will be blocked from submitting any questionnaires in the future.

Our events start at $295.00 and average between $295.00 and $395.00. If you are looking for a budget event (simple delivery of a gift in a theme park), please state this in your questionnaire. The budget events start at $165.00 PLUS the cost of the gift (if any). There are very few budget events. Responding to requests and creating custom ideas is a time consuming process. If you are window shopping and not intend to book an event please do not submit a questionnaire. We will be happy to hear from you when you are ready to commit to a date.

The information you provide by answering the questions below is used ONLY to help us plan a creative event for you. We do not share or sell this information to any person or entity for any purpose and we do not use the information to market to you in the future. Once your event has been planned, the questionnaire is discarded in a manner that will maintain your privacy at all times.

**MAKE SURE THAT YOU ENTER YOUR EMAIL ADDRESS CORRECTLY** If the address entered is invalid your questionnaire will be deleted from our system and we will not attempt to contact you by phone.

 

POLICY FOR SERVICES: There is NO CHARGE for filling out the Pre-Event Questionnaire or for our response. Once you have decided on a custom event and booked your date we will require an initial payment of $100.00. This holds the date for the event and begins purchasing and work for the event. This first payment is nonrefundable but if you need to change the date of the event (if there is still more than 2 weeks before your event date) the whole payment of $100 may be applied to a new available date. The balance is due 10 days before the date of your custom event. We accept VISA, MASTERCARD, AMERICAN EXPRESS, DISCOVER and US Money Orders. We also accept payments made through PAYPAL.COM. If you do not show up at the scheduled time and location for your custom event the entire event price will be forfeited.
